Appointments:
The Glenview Animal Hospital sees patients on an appointment basis. Please call one of our Client Service Representatives and they will be able to schedule an appointment.

Payment Policy:
We appreciate payment when services are rendered. For your convenience, we accept cash, check, MasterCard, Visa and Discover. We do not do any billing.
New Client and Patient information:
The following link will take you to a form that can be printed and completed before your first visit. This information will help us to know your pet a little bit better at your first visit. Please bring this form with you. It is also helpful to have any previous medical records faxed for the doctors to review and to help complete your pet’s medical record.
