Apppointments:
Glenview Animal Hospital sees patients on an appointment basis. Please call one of our Client Service Representatives at 847-724-4812 and they will be able to schedule an appointment.
Our hospital hours are:

Payment Policy:
It is our policy that payment is due at the time services are rendered. For your convenience, we accept cash, check, MasterCard, Visa and Discover. We do not do any billing.
New Client and Patient Information:
The New Client and Patient Information form can be printed and completed before your first visit. This information will help us know your pet a little bit better at your first visit.
Please email or bring this form with you. It is also helpful for us to receive any previous medical records before your appointment. This helps the doctors prepare your pet in a timely way and allows us to complete your pet’s medical record.
